    Schedule:Full-Time or Part-Time | 1 Backup On-Call Shift Per Week | Every 4th-7th Weekend Rotation

    Location:Greater Twin Cities Metro Area | 21-County Service Area (territories assigned based on coverage needs)

    What You'll Do

    • Travel to patients' homes, assisted living communities, hospitals, and skilled nursing facilities to complete hospice admissions.
    • Perform comprehensive clinical eligibility assessments and initiate hospice services.
    • Lead compassionate conversations about goals of care, hospice philosophy, Medicare benefits, and next steps.
    • Build trust with patients, families, physicians, and referral partners from the very first interaction.
    • Coordinate seamless transitions into hospice care with the interdisciplinary team.
    • Complete timely, accurate EMR documentation while maintaining regulatory compliance.

    What You'll Bring

    • Active Minnesota Registered Nurse (RN) license.
    • Experience in hospice preferred, or a strong background in oncology, ICU, palliative care, home health, emergency medicine, or another high-acuity clinical setting.
    • Outstanding communication, assessment, and relationship-building skills.
    • Confidence leading sensitive conversations with patients and families.
    • Ability to work independently in a field-based role.
    • Valid driver's license and reliable transportation.
