    Schedule:Full-Time or Part-Time | 1 Backup On-Call Shift Per Week | Every 4th-7th Weekend Rotation

    Location:Greater Twin Cities Metro Area | 21-County Service Area (territories assigned based on coverage needs)

    What You'll Do

    • Travel to patients' homes, assisted living communities, hospitals, and skilled nursing facilities to complete hospice admissions.
    • Complete comprehensive clinical eligibility assessments and determine hospice appropriateness.
    • Educate patients and families about hospice philosophy, services, Medicare benefits, and what to expect.
    • Coordinate a seamless transition into hospice services.
    • Collaborate with physicians, hospital discharge planners, social workers, intake coordinators, and the interdisciplinary team.
    • Complete timely, accurate EMR documentation while ensuring regulatory compliance.

    What You'll Bring

    • Active Minnesota Registered Nurse (RN) license.
    • Experience in hospice, oncology, ICU, palliative care, home health, emergency medicine, or other complex clinical settings.
    • Exceptional communication, assessment, and relationship-building skills.
    • Confidence discussing hospice care with patients, families, and referral partners.
    • Ability to work independently while managing a flexible field schedule.
    • Valid driver's license and reliable transportation.
