Job Summary:
Center Point Inc. seeks a Counselor/Case Manager for its Male and Woman’s Children Residential Substance Use Disorder Programs in San Rafael, Marin County. The person in this position will be providing outreach, counseling, and case management services to individuals experiencing homelessness or at risk of homelessness.

Overview of the position:
Under the direction of the Program Manager, the Outreach Counselor/Case Manager acts as the lead community services liaison and ensures the delivery of comprehensive case management and therapeutic services for individuals experiencing homelessness or at risk of homelessness.

The Outreach Counselor/Case Manager assists in identifying needed services and ensuring that these services are provided for and/or referrals are facilitated. The Outreach Counselor/Case Manager is responsible for the initial contact, engagement, assessment of needs, and enrollment into the identified assessed services.

The Outreach Counselor/Case Manager provides day-to-day case management activities of the identified individuals experiencing homelessness or at risk of homelessness, addressing linkages to other necessary systems of care (e.g., housing, SUD services, primary medical care, Mental health services, educational and vocational needs).

The Outreach Counselor/Case Manager must demonstrate the ability to establish and maintain cooperative working relationships with staff, colleagues, administrators, and community contacts. Excellent verbal, interpersonal, and written communication skills and the ability to communicate effectively with community constituents from ethnically diverse populations are essential to the position. Ability to plan, coordinate, and implement events, programs, and activities. Demonstrated strong organizational and time management skills and the ability to initiate and problem-solve.

Duties:
  • Assists with establishing eligibility and making applications for public benefits.
  • Responsible for coordinating and implementing outreach and referral services to recruit and secure appropriate treatment placements.
  • Develop case management plans for participants and record in the state and federal electronic records.
  • Make appropriate referrals to internal programs and outside agencies based on the client’s needs.
  • Monitor participants' progress relative to their case management plan.

Minimum Qualifications:
  • High school diploma or equivalent.
  • Have a minimum of two (2) years of experience working with homeless populations experiencing Substance use and or Mental Health disorders.
  • Certified or registered as an AOD Counselor or Peer Support Specialist with a certification recognized by DHCS.
  • Familiarity with Electronic Health Records and online data collection is preferred.
