Who we are:
Community Partners is dedicated to providing quality developmental and mental health services to meet the needs of families and individuals in Strafford County. From comprehensive mental health care, to individualized services for persons with developmental disabilities, to family education and support services, we are here to help. If this resonates with you and sounds like something you’d like to be a part of, we want to hear from you!
About The Role:
We are seeking a dedicated Home Provider for a unique residential opportunity. This role includes housing, featuring a 2-bedroom condominium (to rent and live in) located in Dover, NH, as part of the arrangement.
A Home Providership is a contractual agreement, with a generous stipend, that may be an alternative or supplement to a more traditional employment opportunity.
This is a unique opportunity for you to be a positive role model and mentor, while directly enhancing the quality of another’s life. We believe that the rewards of assisting an individual to grow and become independent can be a wonderful experience for both the individual and the Home Provider.
Our nonprofit agency is dedicated to providing services to adults with developmental disabilities and individuals who are dually diagnosed. We match individuals with interested applicants (Home Providers) who are willing to share their home and life, acting as caregivers, positive role models and mentors. The ability to work with a multidisciplinary team is a must.
Our non-profit organization provides program oversight and offers educational/clinical training and support to ensure success.
We are looking for an individual or couple interested in renting and living in a two-bedroom condo and providing a supportive home environment for an individual as a Home Provider.
We pride ourselves on working with potential home providers and individuals needing homes to find the best match for both home providers and individuals. As a Home Provider you would model positive thinking and provide support around the individual’s physical and metal wellbeing, decision making, accessing community activities and other ADL skills.
Background checks which include, but are not limited to, criminal, OIG, BEAS and motor vehicle records are required.
