Overview:

Shift: Monday - Friday | 8:30 AM - 5:00 PM

 

The Home Medical Equipment Coordinator plays a vital role in supporting exceptional patient care by coordinating new patient referrals and reorders, ensuring medical equipment, supply, and service orders are accurate, complete, and compliant with payer and regulatory requirements. In this fast-paced role, the coordinator serves as a key communication link between referral sources, patients, and the HME operations team, while verifying insurance coverage, obtaining authorizations, and facilitating timely equipment setup. Through strong attention to detail, organization, and customer service, the HME Coordinator helps ensure efficient operations and a seamless patient experience from referral to delivery.

Responsibilities:

· Maintains a broad knowledge in all UnityPoint at Home service lines including, Home Health, Hospice and Infusion services.

· Routinely monitors for new referrals via fax, EMR and other electronic applications.

· Accepts all new referrals and initiates referral documentation in the EMR and ensures accurate information and documentation is included in patient record to facilitate the admissions process, which includes diagnosis codes after validating with RN.

· Secures, confirms and accurately documents provider orders in a timely manner.

· Operates computer system to record/document and retrieve information to ensure appropriate agency services are provided in a timely manner.

Qualifications:

Education:

  • HS Diploma or equivalent with successful completed course work

Minimum Experience:

  • Six months phone based customer service experience. Experience should include high volume, fast paced customer excellence environment such as a call center or similar phone based service role.
