Home Health RN Case Manager

About the Role

A-Line Staffing is seeking a compassionate and organized Home Health RN Case Manager to provide high-quality care coordination for patients in Jeffersonville and surrounding areas. This role is ideal for an RN who excels in clinical assessments, care planning, and patient advocacy, with a focus on improving outcomes in a home health setting.


Coverage Area
  • Harrison County
  • Crawford County
  • Orange County
  • Partial Floyd County
  • Washington County
    Additional coverage may be required for PTO coverage, call-ins, and admissions.

Key Responsibilities
  • Perform comprehensive assessments of high-risk patients
  • Develop and implement individualized care plans
  • Collaborate with physicians, social workers, discharge planners, and interdisciplinary teams
  • Coordinate care transitions and ensure appropriate levels of care
  • Arrange non-medical support services, including transportation and housing
  • Engage specialty resources to optimize patient outcomes
  • Maintain accurate clinical, functional, and fiscal documentation
  • Identify opportunities for health promotion and illness prevention
  • Intervene to prevent adverse patient events
  • Serve as a patient advocate while maintaining privacy and confidentiality

Schedule & Workload
  • Full-Time, Monday–Friday, 8:00 AM – 5:00 PM
  • Weekend on-call rotation required
  • Productivity expectation: 30 points weekly

Qualifications
  • Active, unrestricted RN license in Indiana (Compact license accepted)
  • Associate’s or Bachelor’s Degree in Nursing (or related field)
  • Strong clinical judgment and analytical skills
  • Case Management certification preferred
  • Preferred experience: Wound care, IV care, Drain care, G-tube care, Patient education
  • Homecare Homebase (HCHB) experience preferred
