Key Responsibilities:
  • Patient Assessment & Care Planning: Conducts initial and ongoing head-to-toe assessments, develops individualized care plans, and makes necessary revisions.
  • Skilled Nursing Care: Administers medications, performs wound care, manages IVs, monitors vital signs, and provides treatments.
  • Patient & Family Education: Teaches patients and caregivers how to manage conditions, understand medications, and perform self-care.
  • Coordination: Communicates with physicians, physical therapists, occupational therapists, and social workers for holistic patient care.
  • Documentation: Maintains accurate records of assessments, care provided, and patient progress, often using web-based charting systems. 
Required Skills & Qualifications:
  • Licensure: A current, unrestricted Registered Nurse (RN) license in the state of practice is mandatory.
  • Education: Minimum of an Associate Degree in Nursing (ADN), though a Bachelor of Science in Nursing (BSN) is often preferred.
  • Clinical Experience: Most agencies require at least 1–2 years of experience in acute care or surgical settings.
  • Soft Skills: High levels of autonomy, critical thinking, and communication are essential for working independently in diverse home environments.
  • Technology Proficiency: Experience with electronic medical record (EMR) systems (e.g., Kinnser, HomeCare HomeBase) is standard for mobile documentation.
Work Environment & Benefits:
  • Logistics: A valid driver's license, auto insurance, and reliable transportation are typically required for travel between patient sites.
  • Setting: Variable home environments that may include pets, stairs, and differing socioeconomic conditions.
Essential Skills & Environment: Working Conditions & Environment 
  • Settings: Private homes, assisted living facilities, or residential care settings.
  • Schedule: Often provides flexible hours, but may include mornings, evenings. 
  • Autonomy & Travel: Work independently, traveling between homes, often requiring schedule flexibility.
  • Collaboration: Strong communication skills to work within an interdisciplinary team.
  • Problem-Solving: Differentiate urgent from non-urgent situations and manage varied patient needs.
  • Technical Skills: Proficiency with required technology for documentation and communication. Works independently in various patient homes, often managing multiple patients.
