Position Overview
Provides skilled nursing care to patients in a home health setting across assigned coverage areas. This role focuses on clinical excellence, patient education, and effective care coordination while meeting productivity expectations. The RN will complete OASIS assessments, manage complex clinical needs, and maintain accurate documentation using Homecare Homebase (HCHB).
Schedule & Work Details
Coverage Areas: Portage, Medina, and Summit County
Schedule: Monday through Friday, 8:00am to 4:30pm
Weekly Hour Guarantee: 32 hours
Productivity Expectations: 30 productivity points per week
Pay Rate: $53.00 per hour
On Call Requirement:
One weekday evening per week from 4:30pm to 8:00am
One weekend per month from Friday 4:30pm to Monday 8:00am
Mileage Reimbursement: Yes, reimbursed between visits from first patient to last patient

Key Responsibilities
- Conduct comprehensive home health nursing visits, including Start of Care, Resumption of Care, Recertification, and Discharge visits
- Complete accurate and timely OASIS assessments in compliance with home health regulations
- Utilize Homecare Homebase (HCHB) for documentation and care planning
- Provide skilled nursing services including wound care, wound vacuum management, ostomy care, Foley catheter care, blood draws, and PICC or IV care
- Perform medication management, reconciliation, and patient education
- Educate patients and caregivers on disease processes, medications, and care plans
- Assess patient condition and communicate changes to the interdisciplinary care team
- Maintain compliance with all agency policies, procedures, and regulatory standards
Required Qualifications
- Active RN license
- CPR certification
- Associate's Degree in Nursing
- Previous home health experience
- Demonstrated OASIS knowledge and experience
- Experience using Homecare Homebase (HCHB)
- Clinical experience with wound care, wound vacs, ostomy care, blood draws, PICC or IV care, Foley catheter care, and medication management

Certification and Compliance Requirements
- RN license required
- CPR certification required
