What You'll Do
Assess each new patient's condition, history, and home care needs
Build the care plan with the patient and family
Update the care plan as the patient's status changes
Administer medications and treatments as prescribed
Teach patients and families how to manage care at home
Coordinate with physicians and send updates at least every 60 days
Coordinate with community health partners involved in the patient's care
Plan and carry out discharge when the patient is ready
Arrange any equipment or services the patient needs
Check in on home health aide care every two weeks
Qualifications
Graduate of an accredited school of nursing. One (1) to two (2) years of recent acute care experience in an institutional setting.
Current licensure in state and CPR certification.
Preferred: Bachelor's degree, with one (1) year of home health care experience.
Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
Physical Demands
This role requires the following, with or without reasonable accommodation:
Prolonged walking and standing
Lifting and carrying up to 25 lbs frequently and up to 50 lbs occasionally
Lifting, positioning, and transferring patients (with assistance for patients over 50 lbs, per agency safe-handling policy)
Reaching, stooping, bending, kneeling, and crouching
Visual acuity and hearing sufficient to perform nursing duties
Health Requirements
Required before providing patient care:
Pre-employment physical screening, or a letter from a recent healthcare encounter confirming the candidate is free of communicable disease
Documented negative TB test (TST or TB blood test) taken within the past 12 months
TB risk assessment and symptom evaluation upon hire
