OVERVIEW

InTouch Home Care Inc., is a growing provider of home health nursing and rehabilitation services in 9 counties in and around the San Francisco Bay Area. We are in need of nurses and rehabilitative specialists that can see our patients.

At this time, we are looking for a nurse to see our patients in the South Bay and Peninsula, including San Jose, Santa Clara, Los Altos, Cupertino, Sunnyvale, Palo Alto, San Mateo, Redwood City and the surrounding areas. This position is full-time salaried, which may require work on weekends, but does allow for flexibility as long as a member is meeting their productivity quotas for each week. 

SUMMARY

The nurse plans, organizes, and directs home care services and is experienced in nursing, with an emphasis on community health education. The professional nurse builds from the resources of the community to plan and direct services, in order to meet the needs of individuals, and families with their homes and communities.

ESSENTIAL JOB FUNCTIONS

  • Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness(es).
  • Regularly re-evaluates patient nursing needs.
  • Initiates the plan of care and makes necessary revisions as patient status and needs change.
  • Uses health assessment data to determine nursing diagnosis.
  • Develop a care plan, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive and rehabilitative nursing actions. Includes the patient and the family in the planning process.
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatment as prescribed by the physician.
  • Counsels the patient and family in meeting nursing and related needs.
  • Provides health care instructions to the patient as appropriate peer assessment and plan of care.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
  • Acts as Case Manager and assumes responsibility to coordinate patient care for assigned caseload.
  • May need to draw blood or administer IVs as per patient's needs
  • May need to supervise and/or assess competencies of LVNs and/or other RNs on the team. 

COMMUNICATION

  • Prepares and completes clinical notes in a timely manner and updates the primary physician whenever necessary
  • Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required.
  • Communicates with community health related persons to coordinate the care plan.
  • Maintains open communication via the Agency communication methods (HIPAA Compliant messaging app)
  • Attends regularly scheduled office meetings virtually and/or in-person on an ongoing basis 

ADDITIONAL DUTIES

  • Instructs, supervises and evaluates home health aide care provided every two weeks.
  • Instructs, supervises and evaluates LVN care provided every month (RNs only)

POSITION QUALIFICATIONS

  • Graduate of an accredited school of nursing. One or two years of recent acute care experience in an institutional setting.
  • Current licensure in California and CPR certification.
  • Excellent observation, verbal and written communication skills, problems solving skills, basic math skills; nursing skills per competency checklist.
  • Prolonged or considerable walking or standing. Able to lift, position, or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state/or organization requirements and is in good working order.
  • Candidates with experience in home health preferred.
