Deliver skilled nursing care to patients in their homes while promoting safety, independence, and quality of life.
Complete comprehensive nursing assessments and develop individualized plans of care.
Perform admission, routine, and follow-up visits based on physician orders and patient needs.
Monitor patient progress and identify changes in condition, communicating findings promptly with physicians and the interdisciplinary care team.
Educate patients and caregivers on disease management, medications, treatments, and self-care.
Coordinate care with therapists, aides, physicians, and other healthcare professionals to ensure seamless patient care.
Accurately complete clinical documentation in a timely manner.
Maintain compliance with agency policies, HIPAA regulations, Medicare Conditions of Participation, and professional nursing standards.
Deliver compassionate, patient-centered care that aligns with Elara Caring's mission and values.
