As a therapist with our organization, you will play a vital role in assisting patients to regain their strength, mobility, and independence—all within the comfort of their own homes. By providing tailored care that emphasizes rehabilitation and functional enhancement, you will empower individuals to overcome physical challenges, confidently engage in daily activities, and improve their overall quality of life. You will report directly to the Clinical Manager.

This position is available on a PRN basis, with specific hours and expectations to be discussed during the interview process.

Key Responsibilities
  • Collaborate with physicians and clinical team members to gather patient information and assist in the development, implementation, and modification of therapy treatment plans focused on daily living skills, work-related abilities, community reintegration, adaptive equipment needs, strength, coordination, and range of motion.
  • Engage in care coordination and discharge planning activities, serving as a resource for other healthcare professionals to meet patient needs.
  • Educate staff and family members on managing retraining programs, including skills related to dressing, feeding, grooming, and hygiene, and participate in team and family educational meetings.
  • Prepare and submit evaluations, treatment plans, progress reports, and discharge summaries to supervisors and care management teams.
  • Provide Occupational Therapy Assistants and Home Health Aides with written instructions and care plans that align with the current therapy objectives.
  • Supervise the performance of Occupational Therapy Assistants and Home Health Aides, ensuring proper documentation is completed.
Utilize Your Skills to Make a DifferenceRequired Qualifications
  • Active and unrestricted Occupational Therapy license in North Carolina.
  • Valid driver’s license, auto insurance, and dependable transportation.
  • Current CPR certification.
Preferred Qualifications
  • Experience in home health settings.
Compensation

The pay range for this position is between $54.00 - $76.00 per visit/unit, with an annual base salary ranging from $85,400 - $117,500.

Additional Information
  • Driving Requirements: Participation in our driver safety program necessitates a valid state driver’s license and proof of personal vehicle liability insurance with minimum coverage limits of 100/300/100.
  • TB Screening: As this role involves patient interaction, you will be required to undergo tuberculosis screening if selected.
