Home Health Physical Therapist (PT) PRN

Responsibilities:

• Promotes Elara Carings philosophy, mission statement and administrative policies to ensure quality of care.
• Provides direct patient care, evaluates functional needs and outcomes, and consults with other specialists.
• Designs and implements a plan of care for patients based on thorough assessment and incorporates family participation when appropriate.
• Conducts therapy practice within defined standards of care.
• Directs support staff in achieving patients functional goals.
• Documents evaluations, treatment goals, and plans; regularly maintains documentation and required signatures.
• Provides leadership and acts as a resource to promote physical therapy in the community.

Requirements:

• Graduate of an American Physical Therapy Association (APTA) approved physical therapy program.
• Current, unrestricted PT license in Illinois.
• Minimum one (1) year of experience in a healthcare facility.
• Ability and willingness to travel within the coverage area (up to 50%).
• Valid drivers license, dependable vehicle, and current auto insurance.
• Ability to lift, push, pull, and support up to 50 pounds.
