Bluegrass Orthopaedics is seeking a Full-Time Home Health Licensed Physical Therapist to join our growing therapy team.

At Bluegrass Orthopaedics, our therapy team utilizes evidence-based protocols to drive optimal outcomes across orthopaedic conditions including sports medicine, total joint, hand & upper extremity, and more—now extending that same level of care directly into patients’ homes.

Qualifications

  • Bachelor’s or Doctorate degree in Physical Therapy from an accredited program
  • Licensed Physical Therapist in the state of Kentucky (or eligible)
  • Successful completion of the National Physical Therapy Examination
  • Prior home health or outpatient orthopaedic experience preferred
  • Strong communication and time management skills
  • Valid driver’s license with reliable, insured transportation

 

Responsibilities

  • Provide and direct physical therapy services to patients in their homes, including evaluation, treatment, and care plan development
  • Develop individualized treatment plans with measurable short- and long-term goals
  • Deliver evidence-based interventions to improve mobility, function, and overall quality of life
  • Educate patients and caregivers on home exercise programs and ongoing care strategies
  • Coordinate care with physicians, care teams, and family members to ensure continuity of care
  • Maintain accurate and timely documentation in accordance with company standards and regulatory requirements
  • Supervise and collaborate with Physical Therapist Assistants (PTAs) as appropriate
  • Participate in discharge planning and ensure smooth transitions of care

 

Schedule & Structure

  • Full-time position
  • Flexible scheduling based on patient needs and geographic coverage
  • Primarily field-based role with travel between patient homes
