Qualifications
Bachelor’s or Doctorate degree in Physical Therapy from an accredited program Licensed Physical Therapist in the state of Kentucky (or eligible) Successful completion of the National Physical Therapy Examination Prior home health or outpatient orthopaedic experience preferred Strong communication and time management skills Valid driver’s license with reliable, insured transportation
Responsibilities
Provide and direct physical therapy services to patients in their homes, including evaluation, treatment, and care plan development Develop individualized treatment plans with measurable short- and long-term goals Deliver evidence-based interventions to improve mobility, function, and overall quality of life Educate patients and caregivers on home exercise programs and ongoing care strategies Coordinate care with physicians, care teams, and family members to ensure continuity of care Maintain accurate and timely documentation in accordance with company standards and regulatory requirements Supervise and collaborate with Physical Therapist Assistants (PTAs) as appropriate Participate in discharge planning and ensure smooth transitions of care
Schedule & Structure
Full-time position Flexible scheduling based on patient needs and geographic coverage Primarily field-based role with travel between patient homes
