Home Health & Hospice Physical Therapist (Contract)

Full-Time | 40 Hours Guaranteed Weekly

Coverage Areas: Madison, Montgomery, Clark, Powell Counties

Pay Rate: $50/hr - $53/hr

Schedule & Coverage

• Shift: Monday–Friday, 8:00 AM–4:30 PM EST
• On-Call Requirements:
• • 1 weekend per month
  • 1 weekday per week

About the Role

We are seeking a dedicated and compassionate Physical Therapist (PT) to join our Home Health & Hospice team. This role offers autonomy, meaningful patient relationships, and the opportunity to make a direct impact on patients’ quality of life in their homes. Key Responsibilities

• Start of Care (SOC) assessments
• Comprehensive evaluations and individualized care planning
• Recertifications and ongoing reassessments
• Resumption of Care (ROC) visits
• Agency discharges within PT scope of practice
• Collaborating with interdisciplinary team members to ensure coordinated, patient-centered care
• Documenting timely and accurately in accordance with regulatory and agency standards

Required Qualifications

• Active Physical Therapist (PT) license in the state
• OASIS proficiency
• Experience with HCHB/PointCare or similar EMR systems
• Current BLS certification

Preferred Experience (Nice-to-Haves)

• Prior Home Health experience
• Strong clinical judgment and autonomous decision-making
• Excellent communication and time‑management skills
• Ability to adapt to changing patient needs and home environments
