We are seeking experienced Home Health Physical Therapists to join our growing team in Newport, Calico Rock, Forrest City, and Salem, AR. This role focuses primarily on evaluations, reassessments, and start‑of‑care (SOC) visits, with some routine treatment visits as needed.

Available Locations & Position Details

Newport, AR

• Weekly Hour Guarantee: 32–40 hours
• Productivity Expectation: 30 points/week
• Coverage Area: Within a 50‑mile radius of Newport

Calico Rock, AR

• Weekly Hour Guarantee: 32 hours
• Productivity Expectation: 24 points/week
• Coverage Area: Within a 50‑mile radius of Calico Rock

Forrest City, AR

• Weekly Hour Guarantee: 32–40 hours
• Productivity Expectation: 24–30 points/week
• Coverage Area: Phillips County (Helena / West Helena area)

Salem, AR

• Weekly Hour Guarantee: 32–40 hours
• Productivity Expectation: 24–30 points/week
• Coverage Area: Within a 50‑mile radius of Salem

Schedule & Work Environment

• Schedule: Monday–Friday, 8:00 AM – 4:30 PM
• On‑Call / Weekends: None
• Mileage Reimbursement: Yes — reimbursed between patient visits (from first to last patient)
• Dress Code: Blue scrubs

Key Responsibilities

• Perform PT evaluations, reassessments, and start‑of‑care (SOC) visits for home health patients
• Provide skilled physical therapy treatment visits as appropriate
• Develop and implement individualized care plans focused on functional improvement and patient safety
• Accurately and efficiently complete documentation in compliance with agency standards
• Deliver thorough, high‑quality, patient‑centered care in the home setting

Required Qualifications

• Active Physical Therapist license in the state of Arkansas
• CPR certification
• Prior home health experience
• Experience with Homecare Homebase (HCHB)
