Our dynamic Home Health team is seeking skilled professionals to provide top-quality care to patients in Reeves and Ward counties. What Youll Do:
As a Home Health RN, you'll deliver personalized care to a diverse patient population, managing approximately 7-8 patients during a five-day week or 8-9 patients over a four-day schedule. Your responsibilities include performing start-of-care assessments, Oasis documentation, resumption of care, discharges, and occasional on-call duties once a month. You'll work primarily Monday through Friday, with flexible shift options (8 or 10 hours), and benefit from short travel times between patient locations. Our EMR system is Wellsky, and we currently do hospice care with no hospice patients at this time.
Required Skills:
- Valid RN license in the state
- Proven clinical experience in home health or related settings
- Strong assessment and documentation skills
- Excellent communication and interpersonal abilities
- Ability to work independently and manage a flexible schedule
- Valid drivers license and reliable transportation for local travel
- Ability to handle on-call responsibilities effectively
Nice to Have Skills:
- Experience using Wellsky EMR
- Knowledge of hospice care or palliative care practices
- Bilingual abilities
- Strong organizational skills and attention to detail
Preferred Education and Experience:
- Bachelor of Science in Nursing (BSN) or Associate Degree in Nursing (ADN)
- 1+ years of home health or home care experience preferred
- Certification in Basic Life Support (BLS)
Other Requirements:
- Travel reimbursement provided for travel between patient locations
- On-call availability once a month
- Must be willing to work within the geographic area of Reeves and Ward counties
