Position Details:


  • Locations: Patients' homes in Jersey City and/or Lebanon, NJ
  • Pay: $42.00 - $54.00 per hour
Key Responsibilities:


  • Deliver high-quality nursing care to patients in their homes.
  • Conduct comprehensive health assessments and develop personalized care plans.
  • Administer medications and treatments as prescribed.
  • Monitor patients' progress and adjust care plans as needed.
  • Educate patients and families on condition management and wellness strategies.
  • Collaborate with other healthcare professionals for coordinated care.
  • Maintain accurate and timely documentation of care activities.
Experience & Skills Required:


  • At least 2 years of nursing experience in a home health setting.
  • Expertise in trach and vent care and tube feeding management.
  • Experience working with children and families.
