People over profits. Care over quotas.

At Haven Home Health & Hospice, we recognize that behind every visit, every note, and every moment of care is a healthcare professional making a real difference. This May, we are honoring our Healthcare Heroes, and that starts with how we support our team.

We are a locally owned, clinician-led organization that understands what it takes to do this work well. You are not a number here. We prioritize quality over quantity, giving you the time, support, and autonomy to provide the kind of care you know your patients deserve.

Job Summary:

The Licensed Practical Nurse (LPN) provides skilled nursing services to patients in their homes, supporting the established plan of care. You will assess, implement, document, and monitor patient care while working closely with the RN and interdisciplinary team. Your role is essential in ensuring patients receive consistent, compassionate, and clinically sound care in the place they call home.

What You Can Expect at Haven:

  • Salary Range: $54,600 to $72,800 annually, based on experience
  • PTO and a comprehensive benefits package
  • 401k with company match
  • Company vehicle (pending availability)
  • Flexible scheduling that supports your life outside of work
  • Pet insurance
  • Mental health initiative focused on clinician well-being

Duties and Responsibilities:

  • Recognize and report changes in patient condition to the RN supervisor
  • Complete timely, accurate clinical documentation
  • Support and carry out the patient’s plan of care alongside the RN
  • Collaborate with therapists and other clinicians to ensure continuity of care

Qualifications:

  • Current Missouri LPN license
  • At least one (1) year of recent LPN experience (acute, long-term care, or home health preferred)
  • Home health experience preferred

At Haven, our values guide how we care for patients and how we show up for each other. We are committed to clear communication, accountability, doing what is right, staying forward focused, working together, and continuing to grow.

If you are looking for a place where your work matters and your time is respected, you will find it here.
