Currently we are assisting one of our top home care agency partners with a need for an experienced Home Health LPN to see patients in in homes in the Wake/Johnston County, NC area.



  • Full-time, Monday-Friday
  • On-call rotation (1 full week) with the rest of the nursing staff
  • Competitive pay & mileage reimbursement

Requires a minimum of 1 year current/recent Home Health LPN experience with a Medicare certified agency.




Role and responsibilities include but not limited to:




The Licensed Practical Nurse will provide direct patient care and patient advocacy assistance in the coordination of care through communication with the multidisciplinary team and with the RN Case Manager.




Follows the nursing care plan, established by the case manager, based on nursing diagnoses that included measurable, realistic, and time directed goals and nursing interventions specific to the problem and individual patient.




The LPN reports information about progress or changes to the primary nurse.
