BayCare HomeCare is currently in search of our newest Per Visit Home Health LPN who is passionate about providing outstanding customer service to our community. We are looking for an individual seeking a career opportunity with one of the largest employers within the Tampa Bay area.

Job Details

• Facility: BayCare HomeCare
• Location: Bradenton, FL
• Status: Per Visit PRN
• Shift Hours: 8:00 AM - 5:00 PM
• Weekend Work: Occasional
• On Call: Yes
• Home visits in assigned service area: Manatee County

Job Responsibilities

The Per Visit Home Health LPN provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include:

• Delivery of patient care to assigned patients
• Provides care for clients as ordered, including IV infusions, infusion pumps, and tube feedings under the supervision of a Registered Nurse
• Identifies, implements, and maintains appropriate teaching plans for patient and caregivers under direction of a Registered Nurse
• Follows plan of care and collaborates with Registered Nurse regarding patient response to treatment and change in status
• Utilizes nursing knowledge in the administration of medication and evaluation of effectiveness when applicable

Required Certifications and Licensures

• BLS Basic Life Support
• LPN Registered Nurse License
• Drivers License: State of Florida (required within 30 days of hire)

Education and Experience

• Required: Associates or Diploma in Nursing
• Experience: 1 year Licensed Practical Nurse
• Preferred: 1 year Home Care
