Home Health LPN – Memphis, TN (Shelby County)

Schedule: Monday–Friday, 8:00 AM–4:30 PM 

Weekly Hour Guarantee: 32 hours

On‑Call Requirement: One weekend per month

Pay Rate: $30-32.50/hr

Mileage Reimbursement: Yes — reimbursed between visits, from first patient to last patient 

Dress Code: Scrubs (no color requirement)

We’re seeking a dedicated Licensed Practical Nurse (LPN) to join our home health team serving patients throughout Shelby County. This role is ideal for an LPN who thrives in a field‑based environment, and is passionate about delivering high‑quality, compassionate care directly in the home.

Key Responsibilities:

• Provide skilled nursing care and treatments in the patient’s home as ordered by the physician.
• Educate patients and families on disease processes, medication management, and home‑based care techniques to support independence and safety.
• Document all care accurately and promptly in accordance with agency and regulatory standards.
• Communicate changes in patient condition to the supervising RN and physician to ensure timely interventions.
• Manage and organize patient supplies and medications, ensuring proper storage, usage, and documentation.
• Collaborate with the interdisciplinary team to support patient goals and care plans.

Required Qualifications:

• Active LPN license in the state of Tennessee
• Previous experience in home health, acute care, or a related clinical setting
• Valid driver’s license and reliable transportation
• Strong clinical judgment and ability to work independently

Coverage Area:

This role covers Shelby County, requiring travel between patient homes throughout the day.
