The Health & Hospice RN Case Manager works independently with groups of clients in home health and hospice to provide initial and on-going assessment of the clients skilled needs. Implement individual plans of care and make referrals to others disciplines and/or revise the plan as necessary. Provide ongoing consultation and education for patient/family member regarding all aspects of care. Communicate and collaborate with family /caregivers and other professionals involved in the care of the client. Supervise Licensed Practical Nurses and Home Health Aides.
MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:
Complete initial and ongoing comprehensive assessments of the clients needs, including initiating the
plan of care and revising as necessary
Provide skilled nursing care services and/or treatments requiring substantial and specialized nursing
skill, including intravenous fluids and medications, urinary catheterization, and complex wound care.
Counsel the client and family in meeting nursing and related needs
Initiate appropriate preventive and rehabilitative nursing procedures
Prepare clinical and progress notes
Coordinate services involved in clients care
Make referrals to other services as needed
Inform the physician and other staff of changes in the clients needs
Evaluate outcomes of care and complete the OASIS assessment accurately
Supervise LPN/LVNs and Home Health Aides providing services
Assign home health aides to a specific patient
Participate in in-service programs
Plan for the discharge from service
Comply with agency policies and procedures relative to infection control and safety
EQUIPMENT:
Must have experience with all types of office equipment, i.e. copier, fax, multi-line phone system
PHYSICAL REQUIREMENTS:
Must be able to perform cardiopulmonary resuscitation and relief of an obstructed airway; climb at least
2 flights of stairs and be able to stand and/or walk for a period of at least 4 hours
REQUIRED KNOWLEDGE, SKILLS AND ABILITY:
Current and unrestricted license as a Registered Nurse in the Commonwealth of Virginia
Current CPR certification for healthcare provider
Basic computer skills
Must possess good verbal and written communication skills
Valid Drivers License in the State of Virginia with a minimum age 21 to drive for the organization
Motor Vehicle Driving Record satisfactory to the Agency and its insurance providers
Proof of motor vehicle insurance
