Home Care Physical Therapist Responsibilities
• Evaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy. • Responsible for providing the appropriate level of clinical direction for Physical Therapy Assistants. • Plans implements manages and evaluates the provision of both professional and ancillary home health services direct care and case management to ensure that all patients needs are met and quality care provided in accordance with Federal State JCAHO and program guidelines. • Manages the activities of the multi-disciplinary team providing care to the patient. • Monitors the provision of therapeutic services through written and verbal communication with all disciplines. • Develops implements and carries out a discharge plan in conjunction with the patient family caregiver and members of the health care team. • Interprets agency policy to patients families. • Reviews patients visits weekly and provides plan to Home Care Supervisor and scheduler including anticipated weekend visits. • Informs scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.
Home Care Physical Therapist Requirements
• Bachelors Degree in Physical Therapy • Physical Therapist State of Florida License • 1 year Physical Therapist experience • BLS (Basic Life Support) • Drivers License: Florida drivers license is required within 30 days of hire • Preferred: Masters Degree in Physical Therapy
