Responsibilities
- Conduct patient assessments to identify healthcare needs and develop individualized care plans.
- Provide skilled nursing care, including wound care, medication management, and chronic disease management.
- Collaborate with physicians, therapists, and other healthcare team members to coordinate patient care.
- Educate patients and their families on treatment plans, medication administration, and health maintenance.
- Document patient progress and maintain accurate medical records in compliance with regulatory standards.
- Respond promptly to patient and family inquiries and adjust care plans as needed.
Requirements
- Valid nursing license (RN or LPN) in the state of Michigan.
- Experience in home health care preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and prioritize patient care effectively.
- Reliable transportation to travel to patient homes in Southfield, MI.
- CPR and Basic Life Support (BLS) certification.
