Position Title: Home Health Certified Occupational Therapy Assistant (COTA)

Job Type: Full-Time / Part-Time / PRN

Location: Blacksburg, VA and surrounding areas

Position Summary

We are seeking a compassionate and dependable Home Health Certified Occupational Therapy Assistant (COTA) to join our team. The COTA will work under the supervision of a licensed Occupational Therapist to provide skilled therapy services in patients homes, helping them improve independence and safety with daily activities.

This role is ideal for someone who enjoys building meaningful patient relationships while working independently in the community.

Responsibilities

  • Provide occupational therapy treatments under the direction of the supervising Occupational Therapist and physician orders
  • Assist patients in improving performance with activities of daily living (ADLs), mobility, strength, coordination, and safety awareness
  • Implement individualized treatment plans established by the Occupational Therapist
  • Educate patients and caregivers on adaptive techniques, equipment use, and home safety
  • Observe and report patient progress, changes in condition, and barriers to treatment
  • Complete accurate and timely clinical documentation in the electronic medical record
  • Communicate effectively with physicians, nurses, therapists, and interdisciplinary team members
  • Participate in care coordination and case conferences as needed
  • Maintain compliance with Medicare, state, and agency regulations
  • Travel to patient homes within assigned territory

Qualifications

  • Graduate of an accredited Occupational Therapy Assistant program
  • Current COTA license in the state of Virginia
  • Current CPR certification
  • Home health experience preferred, but not required
  • Valid drivers license, reliable transportation, and auto insurance
  • Strong communication, organization, and time management skills
  • Ability to work independently and provide compassionate patient care
