Job Description:
Join our client’s dedicated team as a Home Health Admission RN. This role is vital in providing high-quality patient care within the home environment across Cumberland, Franklin, Adams, and York Counties. The successful candidate will be adept at managing patient care plans, meeting critical documentation deadlines, and using technology to streamline care delivery.
Primary Duties and Responsibilities:
- Efficiently manage and deliver personalized patient care plans.
- Work collaboratively with the care team to ensure seamless patient experiences.
- Uphold the highest standards of quality in patient care.
- Meet critical deadlines for charting patient encounters.
- Effectively manage time and self-schedule to ensure patient needs are met promptly.
- Utilize technology such as tablets, mobile phones, and electronic medical records software to enhance care coordination.
Qualifications:
- Associate degree in Nursing required.
- Valid RN license in the state of Pennsylvania.
- Experience in home health, hospital, critical care, or acute care preferred.
- Certifications: American Heart Association Basic Life Support (AHA BLS), Advanced Cardiac Life Support (AHA ACLS) preferred.
- Ability to drive with a valid regular Driver License across designated counties.
Schedule and Shift Details:
- 4x10 hours schedule; options include Monday-Thursday or Tuesday-Friday.
- Expected to complete 3 Start of Care (SOC) per day.
