The Home Care Hospice Liaison serves as a community and referral relationship specialist, promoting awareness of hospice and home care services while coordinating referrals from hospitals, physicians, skilled nursing facilities, and community partners. This position helps ensure timely access to compassionate end-of-life and supportive care services through education, outreach, and collaboration with healthcare providers and families.

QUALIFICATIONS


License/Certification
Required: Valid California driver’s license with proof of current automobile insurance and reliable transportation required.
Preferred: Bachelor’s degree in healthcare, marketing, business, or related field preferred. Clinical licensure (RN, LVN, or MSW) preferred but not required depending on role responsibilities and organizational needs.


Education
Required: N/A
Preferred:
Bachelor’s degree in healthcare, business, marketing, or related field 

Experience
Required: Two years of healthcare marketing, sales, or business development for Home Care Services.

Preferred: 
Hospice, home health, or healthcare marketing experience preferred. Established referral relationships and knowledge of hospice services, Medicare guidelines, and community outreach strongly preferred.

Knowledge/Skills/Abilities:
Strong communication and interpersonal skills 
Knowledge of hospice services and Medicare guidelines 
Ability to develop and maintain referral relationships 
Experience with community outreach and territory growth 
Strong organizational and time management skills  

JOB RESPONSIBILITIES
Develop and maintain relationships with referral sources and community partners 
Educate healthcare providers and the community about hospice services 

Generate referrals and support census growth goals 

Conduct routine visits to hospitals, SNFs, assisted living facilities, and physician offices 

Coordinate with the clinical team to ensure smooth patient admissions 

Maintain accurate referral and marketing activity records 

Participate in community events, health fairs, and outreach activities 

Identify new business development opportunities within the assigned territory 

Provide exceptional customer service to patients, families, and referral partners 

Ensure compliance with company policies, HIPAA, and Medicare guidelines
 

Pay Range

$32.00 -$48.00
