Overview:
Shift: Monday - Friday | 8:00 AM - 5:00 PM (+ every 6th weekend)
Provide compassionate care and make a meaningful difference in patients’ lives as a Home Care Hospice Aide. In this role, you’ll deliver high-quality personal care, homemaker services, and companionship to patients in their homes while supporting the RN Case Manager to ensure coordinated, patient-centered care.

Responsibilities:
- Assist patients with daily living activities including bathing, dressing, oral hygiene, and mobility
- Provide light housekeeping, meal preparation, and companionship
- Support medication administration and simple procedures under RN guidance
- Observe and report changes in patient condition or home environment to the RN Case Manager
- Maintain accurate documentation and ensure compliance with care plans and regulatory standards
- Participate in interdisciplinary team meetings, in-services, and quality improvement initiatives
- Build strong, compassionate relationships with patients, families, and caregivers while maintaining professional boundaries
Qualifications:
Education:
- Completion of a 120-hour Nursing Assistant course.
- High School Diploma/GED High School diploma or GED
License(s)/Certification(s):
- Valid Driver’s License (IA, IL, or WI) – Required upon hire
- Basic Life Support (BLS) Certification – Maintain current certification through the American Heart Association (Required upon hire)
- Certified Nursing Assistant (CNA) – State of Iowa (IADIA) or Illinois (IDPH) preferred
- Child Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire
- Adult Dependent Abuse Mandatory Reporter Training – Iowa or Illinois, required within 90 days of hire
Experience:
- Completing of a Nursing Assistant Training Program of a minimum of 75 hours with no continuous period of 24 months without compensation for Home Care or Hospice Aide/CNA work after completion
