Schedule: Full Time | 40 hours per week | Monday - Friday | 8:00 am - 4:30 pm

What You'll Do In This Role:
Be the link that brings care home. As a Home Care Coordinator at LifeCircles, you’ll partner with the interdisciplinary team to assess needs and coordinate essential in-home services that help participants live safely and independently. From arranging durable medical equipment and supplies to organizing personal care, homemaking, respite, and community living supports, you’ll ensure services are delivered efficiently and seamlessly. Your attention to detail and collaborative approach will help create personalized care experiences that truly make a difference in the lives of those we serve. 

Key Responsibilities


Participant Needs Assessment and Care Planning
  • Conduct initial and periodic assessments of participant needs in conjunction with the Interdisciplinary Team (IDT).
  • Recommend interventions to ensure participant function is maximized in the home setting.
  • Develop and coordinate an emergency plan of care in the home setting with the IDT.
Home Care Coordination
  • Arrange assistance for Activities of Daily Living (ADL) and Instrumental Activities of Daily Living (IADL) deficits in the home environment.
  • Arrange home respite care to alleviate caregiver stress.
  • Coordinate and provide personal care supplies and DME.
  • Provide medication reminders in the home setting as directed.
Therapy & Equipment Coordination
  • Work with Physical and Occupational Therapy to ensure participants and/or caregivers understand durable medical equipment.
  • Facilitate coordination of home exercise programs with therapy and home health aides.
Home Visits & Monitoring
  • Conduct regular home visits to ensure services align with the participant's functional goals and meet the needs of the caregiver.
  • Monitor the effectiveness and appropriateness of the care plan, making necessary revisions as conditions change. 
Communication & Collaboration
  • Regularly communicate the appropriateness of the care plan to the IDT.
  • Facilitate bi-directional communication of the care plan to ensure the agency understands the plan and its requirements.
  • Monitor and ensure that the care plan is being followed.
Technology Utilization & Coordination
  • Effectively utilize technology and software to carry out duties, monitor care, and coordinate services.

Skills, Knowledge & Expertise


  • 3 years of experience working in a geriatric setting.
  • 1-year experience working with the frail or elderly population.
  • Home care, administrative, and case management experience.
  • An associates degree in a human service related field.
  • Must have reliable transportation and a current valid Michigan driver’s license.
  • Possess current CPR certification by AHA or willing and able to obtain as offered by LifeCircles.
