Job Title: Care Manager Location: Finger Lakes Region, NY Employment Type: Full-Time Job Summary: The Care Manager is responsible for providing comprehensive care management and coordination for individuals with co-occurring chronic health and behavioral health conditions. This role involves integrating medical, behavioral health, and social services to ensure individuals receive the necessary care and support. Candidates must have their own reliable vehicle for this position. Must-Have Qualifications & Skills: Associate’s Degree in Health or Human Services. Valid NYS Driver’s License. At least one year of experience working with recovery supports, community resources, housing, employment, and other professional and non-professional services. Experience in a behavioral health care setting. Excellent written and verbal communication skills. Ability to multitask and demonstrate composure in challenging situations. Nice-to-Have Qualifications: Bachelor’s Degree or higher in Health or Human Services. 2+ years of experience in Behavioral Health. Experience in Crisis Management.
