Job Description
The Heart Failure (HF) Navigator functions as an advocate for patients diagnosed with Heart Failure. The primary job function is to act as an expert on Heart Failure services and a liaison between our patients, providers and ancillary services in both the inpatient and outpatient settings. The HF navigator analyzes information and take actions for facilitating health and wellness of the patient. This role nurtures working relationships with medical director, physicians, nursing staff, and other disciplines involved in treatment or support of patients.

Completes patient information analysis:
Performs patient observation and monitoring:
Coordinates scheduling and facilitation of testing, appointments with providers:
Promotes patient wellness and facilitates healing:
Promotes decreased readmission by creating an environment that provides access to care.
Instills confidence in the patient's own ability to regain/optimize their own health.
Assists and participates in quality improvement and accreditation projects.
Education
Bachelor's in Nursing (BSN) required.
Master's in Nursing (MSN) preferred.
Experience
3-5 years of cardiac experience preferred.
Certification & Licensure
Registered Nurse with current Virginia license required.
Heart failure certification preferred.
BLS Certification (Basic Life Support) - American Heart 'Healthcare Provider' (HCP) - AHA approved required.
Qualifications
Excellent verbal and written communication skills required.
Ability to encourage team environment through navigation processes required.
Ability to provide education required.
Ability to act as liaison between our patients, providers and ancillary services in both the inpatient and outpatient settings required.
