The Healthy Steps Specialist is responsible for comprehensive coordination of medical care (linkage to supportive services and retention in care) to improve health outcomes for patients of Evergreen Health Primary and Specialty Care. They will provide interventions, screenings,psychotherapyand follow up for families and pediatric patients from age 0 to 5. The Healthy Steps specialist will build strong relationships with families and providers to increase the efficiency of the medical system, and support team-based comprehensive care.
As part of the essential functions of this role: The Healthy Steps Specialist:
Qualified candidate will have a Master's degree in social work, NYS LMSW license, with knowledge and experience in early childhood growth and development, parent-child relational health, infant and early childhood mental health, and/or family systems; excellent written and verbal communication skills; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential

Education/Experience: Master's degree in social work, LMSW license, and experience with early childhood growth and development, parent-child relational health, infant and early childhood mental health, and/or family systems.
