Health Home Care Coordinator- Vietnamese English

Role Scope

As a Health Home Care Coordinator - Vietnamese English, you will support members with chronic conditions by delivering patient-centered care coordination and reducing gaps across medical, behavioral health, and Social Services. Apply www.kwacares.org/careers

This onsite, full-time role in Tacoma, WA (Mon-Fri, 8:30am-5pm; $21.00-$23.00) ensures clients receive the right care at the right time through a structured Health Action Plan (HAP) and ongoing Case Management.

Key responsibilities include care planning; interdisciplinary collaboration across care settings; care transitions using the Coleman Model/Care Transition Intervention (CTI); evidence-based screenings (BMI Assessment, PHQ-9, GAD-7, Katz ADL); Patient Activation Measure (PAM); referrals to community resources and Long-Term Services and Supports (LTSS); client advocacy; and accurate, timely documentation aligned with healthcare regulations and client confidentiality/HIPAA compliance.

Qualifications

  • Bilingual Vietnamese and English (Vietnamese Speaking)
  • AAS degree; 1+ year Behavioral Health Care Coordinator, case manager, or social service coordinator experience
  • WA State Driver's License, auto insurance, and ability to pass DSHS/ALTSA background check

A Typical Workday

Your day as a Health Home Care Coordinator begins by reviewing your caseload, prior notes, and upcoming touchpoints, then preparing for a monthly in-person visit with a Vietnamese Speaking member. You'll meet in the office and out in the community, using interviewing skills to confirm goals, barriers, and progress, and updating the Health Action Plan (HAP) accordingly. Between visits, you connect with clinics, behavioral health partners, and community agencies to keep services aligned and timely, with an emphasis on patient-centered care and smooth care planning. You'll set aside focused time to complete required documentation by monthly deadlines, coordinate follow-ups after transitions in care, and respond to client questions with clear, culturally responsive communication. The day ends with organized case notes, scheduled next steps, and readiness for team check-ins or training.
