Collaborates with health department staff in support of a holistic approach to wellness for Native American clients. Develops, coordinates, and administers culturally sensitive health education programs and materials for individuals and/or groups for prevention and treatment of chronic disease including Diabetes, Obesity, Cardiovascular disease. Duties: ESSENTIAL DUTIES AND RESPONSIBILITIES: Provide excellent customer service to clients and vendors and fellow employees, and represent LTBB Health Department in professional, positive manner. Recognizes and is sensitive to the cultural dimension inherent in the practice of health education in Native American communities. Will serve as the diabetes coordinator/project director for the Special Diabetes Program for Indians grant. Supports health education needs assessment and assessment of resources. Utilizes an interdisciplinary health care team approach to determine the appropriate focus for health education services. Under the direction of the Community Health Manager, create and implement health education programming for the prevention and treatment of chronic diseases. Audiences will include individual patients, groups of patients, families, and health care providers. Programming will include diabetes prevention, nutrition, and lifestyle. Assist in the planning, development, and implementation of individual and group diabetes education strategies for patients through their life span and their families and caregivers. Provide diabetes and self-management guidance and education to pre-diabetics, type 1, type2, and gestational diabetic patients. Individualized education plans shall consider different cultural, physical, cognitive, and literacy levels of patients and families and meet evolving needs of patient. Plans and implements holistic health education programs addressing spiritual and cultural aspects of health. Utilizes and incorporates professional standards and guidelines into program development, implementation, and evaluation. Prepares and submits monthly and quarterly reports to the supervisor as requested, including grant reports. Is responsible for managing the LTBB Diabetes Registry and performing annual audits of both diabetic clients and prediabetic clients. Participates and assists with Community Health events as needed. Collaborates with the Community Health Program to meet program objectives. Maintains basic reference library, educational materials, and audio-visual aids as resources allow. Follows HIPAA and maintains client confidentiality. Provides health education in-service training for staff as needed. Other duties as assigned within scope of the position. Qualifications: EDUCATION AND EXPERIENCE: In addition to KSAs below, REQUIRED: Must have a Bachelors Degree in Health Education, Public Health, Nursing, Social Work, Dietetics/Nutrition, or Kinesiology. Masters Degree preferred. Minimum two years of experience in community health outreach environment related to community health education; other health experience may be considered, depending on scope of work. Previous experience working with the Native American community preferred. KNOWLEDGE, SKILLS, AND ABILITIES: Professional knowledge and skill as a diabetes and health educator. Ability to investigate and analyze information and to draw conclusions. In-depth knowledge and understanding of the nature of diabetes and related issues and patient care concerns. Ability to design, develop, deliver, and evaluate educational programs and materials. Must be able to travel inside and outside of the state- may include flying. Occasional evenings, weekends, and extended overnight work will be required. Moderate walking, bending, and lifting. Must be computer literate and possess excellent written, verbal, and interpersonal communication skills. Must be able to relate to Native and Non-Native Community Members. Must be a collaborator; able to establish and maintain effective working relations with the general public, other agencies, and co-workers. Must be able to establish rapport with the physicians, nurses, members of the primary care team, clients, and associates. Must be able to give and accept constructive criticism related to job or job performance. Will be expected to contribute to the overall growth and development of the Community Health program. Must be able to work without direct supervision. Experience with EPIC EHR software preferred. CERTIFICATES, LICENSES, REGISTRATIONS: Must possess a valid driver''s license, reliable transportation and have an insurable driving record for duration of employment. Must maintain license and/or certification if applicable. COMMENTS: Indian preference will apply (Preference will be given to qualified Native American applicants in accordance with the LTBB Indian Preference Statute). Individuals must pass a thorough background investigation, including criminal and employment. Individual must have a positive job history with former employers, including LTBB.
