**Job Summary:**
We are seeking a Healthy Start Visitor to join the Health Department team. This role is responsible provides family-centered, home-based services to pregnant individuals, infants, and families with young children by promoting healthy pregnancies, positive parenting practices, child development, and family self-sufficiency by conducting regular home visits, providing education, assessing family needs, connecting families with community resources, and supporting overall maternal and child health outcomes.
**Qualifications:**
- High school diploma or equivalent required
- 1 year of work experience or other relevant experience
- Valid driver's license required or attainable within 30 days of hire
- Excellent communication and interpersonal skills, in both English and Spanish
- Ability to multitask and work in a fast-paced environment
- Sensitivity to diverse cultural backgrounds
**Responsibilities:**
- Must be able to maintain HIPPA compliance
- Completes comprehensive family assessments and develops individualized family plans
- Coordinates referrals to medical providers, social service agencies, behavioral health providers, and community resources
- Enforces and maintains compliance with all federal, state and local laws and ordinances
- Follow privacy regulations and maintain confidentiality of sensitive information
