Healthy Families New York (HFNY), a Healthy Families America (HFA) accredited home visiting program, seeks to improve the health and well-being of infants and children through home-based services delivered by non-profit organizations in local communities. HFNY was established in 1995 by the New York State Office of Children and Family Services (OCFS).

HFNY is an evidence-based voluntary home visiting model designed to provide services to families that begin prenatally, or at birth, through age five. HFNY uses an infant mental health/relational development approach that promotes parent-child attachment to achieve its mission of preventing child abuse, neglect, and other adverse childhood outcomes.

Position Overview:
The Health Coach supports the Comparing Health Interventions for Maternal Equity (CHIME) Study, a five-year maternal health research initiative in partnership with Stanford University. The position provides health coaching, case management, home visiting, and health education services to pregnant and postpartum participants, promoting healthy lifestyles and reducing barriers to maternal health.

Each Health Coach manages a caseload of up to 30 participants, providing individualized support from pregnancy through 12 months postpartum.
Key Responsibilities:
  • Provide health education and home visiting services throughout pregnancy and postpartum
  • Facilitate curriculum-based health education workshops
  • Coach participants on nutrition, physical activity, and healthy lifestyle goals
  • Conduct social needs screenings, assessments, and referrals
  • Meet with participants weekly at home, in the office, or within the community
  • Maintain detailed case notes and participant documentation
  • Track participant enrollment, engagement, and study progress
  • Provide ongoing support through in-person visits, text messaging, and referrals
  • Collaborate with program leadership, research staff, and consultants
Requirements:
  • Bachelor's degree in Public Health, Community Health, Social Work, Nutrition, Counseling, or related field (preferred)
  • 1–2 years of relevant experience
  • Experience with community outreach, case management, health education, or wraparound services preferred
  • Strong communication, presentation, and interpersonal skills
  • Excellent organization and documentation abilities
  • Comfortable conducting home visits throughout NYC
  • Must reside in NY, NJ, or CT
  • Bilingual English/Spanish or English/Haitian Creole preferred
  • Demonstrated interest in maternal health, reproductive justice, racial equity, and community health
Pay Range: USD $52,000.00 - USD $52,000.00 /Yr.
