Job Summary

Responsible for providing education and coaching regarding healthy lifestyle practices and chronic health conditions. Also responsible for providing one-on-one wellness and health coaching, as well as providing support with our cloud-based platform. Works with individuals to assist in the identification and enhancement of individual motivations for pursuing wellness with the goal of pursuing behavior changes conducive to improved lifestyle and health. May assist in Student Interns oversight.

Essential Functions

  • Conduct individual assessments to identify health risks, set goals, and develop customized health and wellness plans.
  • Develop a program delivery plan and implementation schedule.
  • Prepare reports of activities and program outcomes.
  • Monitor preexisting health conditions of clients.
  • Assist with and implement programs to improve health outcomes.
  • Preparing and maintaining technical documentation of data and analysis files.
  • Create and prepare reports and presentations.

Bachelor's Degree Related Field of Study required

Licenses and Credentials

HeartSaver CPR AED - American Heart Association (AHA) required Health Coach Certification (NSHCOA) - National Society of Health Coaches (NSHCOA) preferred Health and Wellness Coach [NBC-HWC] - The National Board of Health and Wellness Coaching (NBHWC) preferred

Experience

Experience in health promotions/education setting, counseling on health topics such as nutrition, exercise, cholesterol, hypertension, stress, alcohol, and smoking 2-3 years preferred

Knowledge, Skills and Abilities

  • Requires the ability to work independently as well as function effectively within a team-based model of care.
  • Excellent oral and written communication skills.
  • Competence in Microsoft Word, Excel and PowerPoint.
  • Ability to adapt to program evolution and shifts in prioritization.
  • Ability to plan, implement and evaluate disease prevention and health promotional programs.
  • Excellent interpersonal skills and ability to work effectively with physicians and their staff in a physician practice setting.

This role is aimed towards people either family or friends of those who are or have struggled with addiction instead of those who have personally battled addiction.

The CPR certification can be gained through the MGB training and is not necessary prior to starting.
