Ultimate Staffing is actively seeking a dedicated, fully-bilingual (Spanish/English) Health Case Manager to join a mission‑driven healthcare organization in Greeley, CO. This role provides intensive, client‑centered case management to individuals with complex medical, behavioral health, and social needs. Responsibilities
- Assess, evaluate, and reassess client needs to develop and maintain individualized care or wellness plans.
- Provide ongoing medical case management support to clients navigating chronic health conditions and life‑stability challenges.
- Coordinate care with healthcare providers, behavioral health professionals, housing programs, and community agencies.
- Support clients with medical engagement, treatment adherence, benefits access, and basic needs coordination.
- Advocate for client needs while empowering individuals to actively participate in their care plans.
- Monitor client progress and adjust care strategies in collaboration with multidisciplinary teams.
- Maintain thorough, accurate, and timely documentation in electronic health and case management systems.
- Provide education, guidance, and emotional support to clients in a respectful, trauma‑informed manner.
- Participate in case reviews, care conferences, and continuous improvement activities.
- Ensure all services align with healthcare regulations, program guidelines, and organizational standards.
Qualifications:
- Bachelor's degree in Nursing, Human Services, Social Work, or related field required.
- Bilingual Spanish/English required.
- Registered Nurse (RN) license or relevant certification preferred.
- 2+ years of experience in case management or a related healthcare role.
- Strong interpersonal and communication skills.
- Proficiency in electronic health record (EHR) systems.
- Ability to work independently and as part of a team.
- Detail-oriented with excellent problem-solving abilities.
- Understanding of healthcare regulations and patient rights.
Required Work Hours:
9:00 AM - 5:00 PM, Monday - Friday; Onsite
