About Pueblo Regional Center:

The Pueblo Regional Center (PRC) is one of three state-operated regional centers providing quality, specialized services and supports to individuals with developmental disabilities and mental health diagnoses. PRC manages 11 group homes throughout the Pueblo West community, licensed to serve 88 individuals. PRC provides 24/7 care and treatment in home-like settings, while offering habilitative services and supports in the areas of residential, day programming, therapies, and health and nutrition. Supervision over this position will come from the Program Services Coordinator (PSC).

Description of Job

About the Position:

This position ensures all residential programming is implemented in the home. This includes providing supervision, modeling, and coaching to direct support professionals. This position spends the majority of the time in the home ensuring the milieu of the home is operating to efficiency. This position works closely with the PSC to ensure the implementation of services is occurring within the correct guidelines.

Primary Job Duties:

Individual Care/Communication/Unit Maintenance/Safety. Understands, promotes, and participates in the Home and Community Based Services Final Rule. Counsels individuals on behavior and medical care needs during group or one on one interactions per ID team direction. Collaborates with all investigations and other authorities as needed. Administer medications and treatments in accordance with physician order and Psychiatric Technician Act/Licensed Practical Nurse Practice Act. Can be pulled to cover any home/shift in the organization. Can be transferred to any home/shift in the organization to meet agency needs. Demonstrates knowledge of behavioral and therapeutic approaches. Advises individuals regarding self-care and problem solving. Provides supports, services and tools which will assist the individual in the determination of personal goals, as well as foster the maximization of personal growth, independence and productivity. Completes and turns in paperwork to include incident reports, injury on the job reports, debriefing paperwork, progress notes etc. Utilizes communication devices/cell phones, computers, electronics, inter/intranet, etc. correctly and per policies outlined by the Pueblo Regional Center. Addresses all complaints and resolving individual grievances at the home level. When a resident is placed in an agency-approved physical hold, follows policy, competencies, and procedure. Maintains strong and respectful communication skills between co-workers, management, individuals, and outside agencies/stakeholders. Corrects deficiencies as needed in a timely manner. Maintains the home in perpetual readiness for surveys. Maintains current knowledge and complies with policy and procedure in these areas. Attends mandatory meetings, to include supervisory meetings and house meetings, monthly and quarterly reviews.

Clinical Skill. Implements and trains in the following areas: physically lift, transport and/or position residents by using approved lifting techniques. Employs agency-approved verbal and physical techniques in addressing a behaviorally challenging resident. Documents outcomes of treatments and program implementation timely and accurately. Monitors and reports progress, maintenance and regression according to IP and ISSP. Safely operate a motor vehicle when transporting individuals to locations. Ensures safe transportation of individuals to work area, program area, medical appointments, court hearings and community activities, as well as taking an active part in vocational training. Assists individual with personal shopping; assists in ordering/purchasing supplies for the group home; follows Agency/State rules and policies and procedures; assures legal compliance with all purchases; and maintains agency mission. Participates in Interdisciplinary Team process, discussing individuals' ISSPs (through goal and objective training, agency-approved verbal and physical techniques, direct contact and monitoring). Formulation of accurate information for assessments and formal reports for inclusion in the team process. Communicates to EOC, PSC, and RN regarding individual medical/behavior needs or changes. Ensures confidentiality of the medical record and information, conforming to HIPAA practices and procedures.

Supervision. Exercise supervision over the assigned home direct care staff (approx. 12), in the area of hiring, performance planning, performance evaluation and potential disciplinary actions. Works collaboratively with the scheduling department and implements regulatory compliance in conjunction with: DPA regulations; Personnel Rules; State Board of Nursing Practice Acts; HCBS-DD Waiver; Health Facilities and Emergency Medical Services Division (HFEMSD) of Department of Public Health (DPH); Centers for Medicare and Medicaid (CMS); CDHS Policies and Procedures; and other state and federal mandates. Manages staffing patterns to ensure a balance of all agency staff to meet the needs of the residents, work units and agency as a. Directly monitors, trains and observes direct report staff in areas of residential, day program and therapy needs; documents accordingly. Must be able to drive to work areas to provide this supervision and ancillary duties. Will formalize a supervision schedule, to provide objective feedback for professional growth. Will complete with accuracy Performance Management Plan with steps for professional improvements.

Fully understand all aspects of fundamental rights, including the right to be free from restraints and free of abuse; Responsible for promptly reporting to the appropriate administrative staff incidents or evidence of abuse or neglect. Position will communicate and interact positively and effectively with each person, visitors, family members, guardian, peers, and supervisors; and support an atmosphere which promotes a home environment.

Will have knowledge and ability to utilize and follow: Pharmacy Formulary, Treatment Plans, Infection Control Manual, Physicians Orders, Court orders, Environment of Care Manual, Centers for Medicare and Medicaid Services, Safety Data Sheets, Colorado State Personnel Procedure Manual, Affirmative Action policy, Colorado Workers Compensation guidelines, Emergency Guides, ADA statutes, Colorado Alliance for the Mentally Ill recommendations, Residents Rights. Responsible for attending in-service training to meet own educational and mandatory training requirements-keeping current. Maintains required licensure and certifications. Responsible to remain current in mandatory trainings, computer based trainings, rosters, policies, procedures.

Quality Assurance Performance Improvement (QAPI). Maintains and follows Pueblo Regional Center's procedures/forms/form instructions/resource sheets/computerized trainings/nursing groups/competencies/staff training. Participates in the QAPI process at the home and day program as directed by your leadership team. Utilizes the most current version of forms, policies, procedures, addendums, and form instructions. Ensures timely, accurate resident care documentation and participates in the QAPI process at the house level as directed by your supervisor. Documents changes in individuals' condition and significant events identified in team notes, ensures medical conditions are identified and followed up occurs, follow the individual support plans and well as the individual behavior support plans. Documents all information objectively and accurately. Provides documentation that is descriptive, objective and person centered focused. Reflects the reason for admission, individual's goals, interventions and outcomes are apparent. Will follow up and correct errors. Correctly perform transcription, utilize the correct and most current version of forms, follows policies/procedures/addendum/form/form instructions/ State Board of Nursing/Psychiatric Technician Act/ Licensed Practical Nurse Practice Act.
