CO State Veterans Community Living Center at Rifle
The Colorado State Veterans Community Living Center at Rifle is located on Colorados western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including a 18-bed Special Care Unit for residents with Alzheimers Disease or related conditions. Serving veterans, their spouse and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. All staff members of Rifle are committed to providing a life of qualify and dignity for every resident. At the Colorado State Veterans Community Living Center at Rifle, our staff is known for showing love and compassion to our residents. Giving residents a hug, holding their hand or sitting next to them on them on the arm of a chair are just a few of the ways we give care that comes from the heart.
Description of Job
You are required to work at your assigned work location during your scheduled work times.
Monday thru Friday, 8:00 am - 4:30 pm

The position exists to assist the Medical Records Technician in performing all functions of the health record program. This position schedules all health care appointments and coordinates the transportation schedule, as well as provide notification to family/resident of appointments. Follows up with physician offices to obtain reports for records. Maintains census reporting in absence of Medical Records Technician. Tracks physician visits (Regulatory and History & Physicals) needed to be performed by physicians on a monthly basis

Infection Control:
Provide infection control, implementing/using universal precautions, disposal of waste. Clean linen room and equipment, stock supplies, complete supply forms.

Education/Licensure/Certification/Experience:
A valid nurse aide certification (CNA) from the Colorado Department of Regulatory Agencies for the high school diploma or GED ~or~ A valid practical nursing license (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact for the nurse aide certification (CNA) can be substituted for the education and work experience
At least one (1) year of professional experience as a CNA or performing similar duties/assignments of a CNA.
Must have valid Drivers license
Substitutions:
Current, valid certification as a Nurse Aide (CNA) from the Colorado Department of
Regulatory Agencies will substitute for the high school diploma or GED

Previous experience transporting patients, residents, or individuals in a healthcare, long-term care, or rehabilitation setting.
Experience providing excellent customer service while interacting with patients, residents, families, and staff.
Knowledge of safe patient handling and transfer techniques.
Experience operating wheelchairs, stretchers, lifts, and other patient transport equipment.
Ability to effectively communicate with individuals from diverse backgrounds, including those with cognitive or physical impairments.
Strong organizational skills and ability to prioritize multiple transport requests.
Demonstrated ability to work independently and as part of a team.
Basic computer skills for documenting and utilizing electronic medical records.
Experience working with older adults, veterans, or individuals with disabilities.
Excellent attendance record and demonstrated reliability
Experience working in a skilled nursing facility, assisted living, or veterans' home.
Knowledge of infection prevention and control practices.
Commitment to providing person-centered, compassionate care while maintaining resident dignity and privacy.
Experience supporting emergency preparedness procedures and safe resident evacuation when needed.
