The clinician will oversee medication management, including safe prescribing and adjustment of medications to minimize polypharmacy complications. Care coordination duties encompass managing patient transitions between hospitals, rehabilitation centers, and home care settings. No call coverage is required for this assignment, and board certification and prescriptive authority are required.
