Matriarch Home Care Concierge is growing, and we are looking for a driven, compassionate RN who wants to do more than traditional nursing. This role blends clinical expertise, care coordination, leadership, and community engagement to help families create personalized care solutions.

Responsibilities

As our RN Concierge Care Manager, you will serve as a trusted guide for families and clients across the Greater Nashville area.

Responsibilities include:

• Conduct in-home nursing assessments for new and existing clients
• Develop personalized care plans tailored to each client's needs and goals
• Provide oversight and support to concierge care specialists (skilled and non-skilled staff)
• Coordinate services and communication between families, providers, and care teams
• Serve as a resource and advocate for clients and their families
• Participate in community networking events to build relationships and grow the concierge program
• Support caregiver development, mentorship, and team growth
• Help refine processes and contribute ideas to help scale a growing concierge care model

Requirements

• Active Registered Nurse (RN) license in Tennessee
• Experience in home care, case management, or geriatric care preferred
• Strong communication and organizational skills
• Ability to travel throughout the Greater Nashville area
• Passion for serving seniors and their families
