Clinicians average 12+ patients a day at a full caseload, and provide treatment modalities including:

• Diagnostic Evaluation
• Medication Management
• rTMS (optional)

This geriatric APP position is a full-time position that operates through assisted living homes in the Richmond, Virginia area partnered with Bloom Health Centers.

Job duties include:

• Perform psychiatric evaluations and ongoing assessments for assisted living residents.
• Diagnose and treat geriatric mood disorders, dementia-related behaviors, psychosis, and anxiety.
• Partner with primary care providers, Directors of Nursing (DONs), and facility teams on resident care plans.
• Complete timely EMR documentation compliant with Virginia state and federal long-term care regulations.

APP Qualifications:

• Active Virginia PMHNP-BC or PA-C license in good standing
• Active or pending Virginia non-exempt DEA
• Microsoft Office proficient
• Understanding of DSM-V and diagnosis codes
• Organizational and time management skills
• Writing and oral communication skills
• Willingness to learn
• Ability to commute to multiple locations
