Social Worker-Boston Children’s Health Physicians – Valhalla, NY The Pediatric Nephrology and Rheumatology Divisions of Boston Children’s Health Physicians have an immediate opening for a full time social worker. Our divisions, based on the campus of Maria Fareri Children’s Hospital at Westchester Medical Center and New York Medical College, have a growing team of 4 physicians dedicated to offering the best clinical care possible, combined with a commitment to education, research, and ongoing quality improvement. Our team taking care of complicated patients including children with chronic kidneys diseases requiring dialysis and kidney transplantation The ideal candidate will contribute to all facets of our programs, including, but not limited to, mental health screening, service referrals, case management, building rapport with the families, helping with social factors such as transportation, housing, insurance, and schooling. We are looking for a hard-working professional who will confer and collaborate with our team, families, community agencies, and assist with the coordination and provision of clinical services. The candidate should feel comfortable advocating for families within various resource systems and navigating service organizations to complete referrals. Individual and/or family counseling for children, adolescents, and adults is encouraged depending on the credentials of the successful candidate. Conversational Spanish is preferred, but not required. Supervision for clinical licensure is available. LMSW or LMHC required. Pay rate will be $25/hr-$39/hr, negotiable based on experience.
