This rewarding opportunity allows nurses to provide compassionate, patient-centered care to individuals in their homes while supporting patients and families during critical moments of care.
The ideal candidate will have strong clinical skills, a compassionate bedside manner, and experience or interest in home health nursing or hospice care. This role offers the opportunity to make a meaningful impact while working in a supportive healthcare environment. Job Responsibilities
Provide skilled nursing care to patients in their homes in accordance with individualized care plans
Assess patient health status, monitor symptoms, and document clinical findings accurately
Administer medications, treatments, and wound care as prescribed by physicians
Provide hospice and palliative care focused on patient comfort, dignity, and quality of life
Educate patients and family members about disease management, medications, and care techniques
Coordinate care with physicians, therapists, social workers, and other healthcare professionals
Monitor patient progress and adjust care plans based on changing needs
Maintain accurate and timely documentation in compliance with regulatory standards
Support patients and families emotionally while delivering compassionate end-of-life care
Participate in interdisciplinary team meetings to discuss patient care plans and progress
Education Requirements
Associate Degree in Nursing or Bachelor of Science in Nursing from an accredited nursing program
Licensure and Certification Requirements
Active Registered Nurse (RN) license or eligibility for licensure in the state of Nebraska
Current Basic Life Support certification
Valid drivers license and reliable transportation for patient home visits
Preferred Qualifications
Previous experience in home health, hospice, palliative care, or community health nursing
Strong patient assessment and care planning skills
Ability to work independently while managing multiple patient visits
Excellent communication and interpersonal skills when working with patients and families
Compassionate approach to patient care with a focus on comfort and dignity
