    Physical Therapist (PT) – Permanent Opportunity | North of Stockton, CA

    Connected Health Care is partnering with a highly respected healthcare organization in a charming community north of Stockton, California, to hire a full-time Physical Therapist.     Responsibilities

    • Evaluate patients and develop individualized treatment plans
    • Provide evidence-based physical therapy services to patients with orthopedic, neurological, post-surgical, and general rehabilitation needs
    • Monitor patient progress and adjust treatment plans as appropriate
    • Educate patients and their families on treatment goals, home exercise programs, and injury prevention
    • Collaborate with physicians, occupational therapists, speech therapists, nursing staff, and other members of the healthcare team
    • Maintain accurate and timely documentation in the electronic medical record
    • Ensure compliance with all regulatory, safety, and organizational standards

    Qualifications

    • Doctor of Physical Therapy (DPT) or Bachelor's/Master's Degree in Physical Therapy from an accredited program
    • Current California Physical Therapist license (or eligibility to obtain)
    • BLS Certification
    • Strong communication and patient education skills
    • Previous outpatient, inpatient, or acute care experience is a plus
