DescriptionABOUT THE ROLEActiva Home Health is expanding its Hospital at Home program, delivering hospital-level care to patients in their homes as an alternative to inpatient hospitalization. Patients remain formally admitted to a hospital partner while receiving coordinated in-home care supported by physician oversight and continuous clinical monitoring.
POSITION SUMMARYProvide hospital-level skilled nursing care to patients enrolled in a Hospital at Home program. RNs deliver high-acuity care in the home while completing timely, accurate clinical documentation in Kinnser (WellSky).
KEY RESPONSIBILITIES
Deliver hospital-level clinical care in the patient's homeComplete timely, accurate, and thorough clinical documentation in Kinnser (WellSky) NO OASIS REQUIRED Communicate patient status changes per established clinical protocolsAdhere to program workflows, response expectations, and professional standardsProvide care that reflects hospital-level quality and accountability
REQUIRED QUALIFICATIONSActive, unrestricted professional licenseStrong assessment, clinical judgment, and documentation skillsAbility to work independently while meeting hospital-level expectationsComfort with mobile technology and electronic documentation systemsReliable transportation and strong attendance record
PREFERRED EXPERIENCEAcute care, hospital, ICU, ER, or step-down experienceHigh-acuity home health or complex medical care experienceExperience collaborating with interdisciplinary clinical teams
