We are looking for an experienced and compassionate Registered Nurse (RN) to join our growing home health team. This hybrid role combines in-home patient visits with remote clinical support, offering flexibility while maintaining a high standard of patient care.


Position Overview


The RN will complete assigned Start of Care (SOC), Admission, Resumption of Care (ROC), Recertification, and Discharge visits in patients' homes. When not conducting field visits, the RN will work remotely, supporting clinical operations, reviewing documentation, coordinating patient care, and assisting the care team.



Field Responsibilities

• Complete up to three (3) Start of Care (SOC),1 Admission, or 1 Resumption or 1 Recertification or 1 Discharge (DC) 
• Perform comprehensive patient assessments and establish plans of care
• Document all visits accurately and in compliance with Medicare and agency guidelines

Remote Responsibilities

• Review and process physician orders
• Complete patient discharge documentation
• Provide clinical guidance and support to field clinicians
• Coordinate patient care with physicians and interdisciplinary team members
• Ensure timely documentation and regulatory compliance

Schedule

• Full-time hybrid position
• Work remotely when not assigned in home OASIS
• Must be available to work every other Saturday
• Wednesday will be your scheduled day off during Saturday work weeks

Qualifications

• Current, unrestricted Registered Nurse (RN) license
• Minimum one year of home health experience preferred
• Experience completing SOCs, Admissions, ROCs, Recertifications, and Discharges strongly preferred
• Experience in Axxess preferred 
• Strong clinical assessment and documentation skills
• Excellent communication, organization, and time management abilities
• Ability to work independently in both field and remote settings
