Pay Rates – Per Completed Visit
Per Visit - $45
Downtime (pre-approved wait time) - $30/hr
$1.000 SIGN ON BONUS
"Completed" = visit done in the home + documentation finished in EMR within 24 hours + orders/signatures uploaded + meets QA standards.

What You'll Do
- Provide skilled nursing care to home health patients in accordance with the established plan of care
- Monitor and document patient symptoms, vital signs, and changes in condition
- Administer medications, wound care, and treatments as directed by the RN Case Manager or physician
- Communicate effectively with patients, families, and the interdisciplinary care team
- Provide patient and family education related to care, medications, and disease management
- Maintain accurate and timely documentation in the EMR within required timeframes

Requirements
Must-Have:
- Active LPN license (Ohio) + current BLS certification
- Reliable vehicle and valid driver's license with ability to travel throughout the Cleveland / Northeast Ohio service area
- Minimum 1 year of clinical nursing experience
Preferred / Nice-to-Have:
- Prior home health experience
- Familiarity with HCHB
- Experience with wound care, IV therapy, or other complex skilled nursing procedures
